A considerable section of the population in India accesses the services of individual private medical practitioners (PMPs) for primary level care. In rural areas, these providers include MBBS doctors, practitioners of alternative systems of medicine, herbalists, indigenous and folk practitioners, compounders and others. This paper describes the profile, knowledge and some practices of the rural doctor in India and then discusses the reasons for lack of equity in health care access in rural areas and possible solutions to the problem.

Optimization by definition is the action of making most effective or the best use of a resource or situation and that is required almost in every field of engineering. In this work, the optimization of Least Mean square (LMS) algorithm is carried out with the help of Particle Swarm Optimization (PSO) and Ant Colony Optimization (ACO). Efforts have been made to find out the advantages and disadvantages of combining gradient based (LMS) algorithm with Swarm Intelligence SI (ACO, PSO). This optimization (...) of LMS algorithm will help us in further extending the uses of adaptive filtering to the system having multi-model error surface that is still a gray area of adaptive filtering. Because the available version of LMS algorithm that plays an important role in adaptive filtering is a gradient based algorithm, that get stuck at the local minima of system with multi-model error surface considering it global minima, resulting in an non-optimized convergence. By virtue of the proposed method we have got a profound solution for the problem associated with system with multimodal error surface. The results depict significant improvements in the performance and displayed fast convergence rate, rather stucking at local minima. Both the SI techniques displayed their own advantage and can be separately combined with LMS algorithm for adaptive filtering. This optimization of LMS algorithm will further help to resolve serious interference and noise issues and holds a very important application in the field of biomedical science. (shrink)

In today’s age, we see the increasing influence of technology on people, which begs to raise the question: “Is society determined by technology?” Rising up within the constraints of each society, technology had its limitations, as it catered to the needs and interests of the masses. As society evolved, so did its requirements. We are at a stage where dependence on technology has gone through the roof with new innovations coming up in the sector, the rise of artificial intelligence, for (...) instance. Technology has replaced the concept of being a mere tool and is creating/demanding space of its own. With advancements in various fields like robotics which churn out machines that can mimic human behaviour and are equipped with anthropomorphic features, it has become rather difficult to view them as mere commodities waiting for one’s (human master’s) command. Keeping this in view, the paper primarily focuses on the relationship between society and technology, with special emphasis on artificial intelligence, through an analysis of the films Ra One (Bollywood), Android Kunjappan (Mollywood), and Ironman 3 (Hollywood). Secondarily, an experimental study was carried out to know the perception of the Engineering students, the future technologists, who are the stakeholders of the 21st-century technological world, with an aim to know their perception of technology and bring an awareness of the limitations of technology and its role in the creation of utopian and dystopian world. Finally, the paper examines the consequence of technological determinism and autonomy from a colonial perspective and intends to bring preparedness among the students to perceive technology conscious of its limitation and bring harmony between society and technology that leads towards building a utopian world. Best practices in ontology building for biomedicine have been frequently discussed in recent years. However there is a range of seemingly disparate views represented by experts in the field. These views not only reflect the different uses to which ontologies are put, but also the experiences and disciplinary background of these experts themselves. We asked six questions related (...) to biomedical ontologies to what we believe is a representative sample of ontologists in the biomedical field and came to a number conclusions which we believe can help provide an insight into the practical problems which ontology builders face today. Rice cultivation is one of the most important agricultural activities on earth, with nearly 90% of it being produced in Asia. It belongs to the family of crops that includes wheat, maize and barley, and it supplies more than 50% of calories consumed by the world population. Its immense economic value and a relatively small genome size makes it a focal point for scientific investigations, so much so that four whole genome sequence drafts with varying qualities have been generated by (...) both public and privately funded ventures. The availability of a complete and high‐quality map‐based sequence has provided the opportunity to study genome organization and evolution. Most importantly, the order and identity of 37,544 genes of rice have been unraveled. The sequence provides the required ingredients for functional genomics and molecular breeding programs aimed at unraveling intricate cellular processes and improving rice productivity. Introduction of deep neural networks to the machine translation research ameliorated conventional machine translation systems in multiple ways, specifically in terms of translation quality. The ability of deep neural networks to learn a sensible representation of words is one of the major reasons for this improvement. Despite machine translation using deep neural architecture is showing state-of-the-art results in translating European languages, we cannot directly apply these algorithms in Indian languages mainly because of two reasons: unavailability of the good corpus and (...) Indian languages are morphologically rich. In this paper, we propose a neural machine translation (NMT) system for four language pairs: English–Malayalam, English–Hindi, English–Tamil, and English–Punjabi. We also collected sentences from different sources and cleaned them to make four parallel corpora for each of the language pairs, and then used them to model the translation system. The encoder network in the NMT architecture was designed with long short-term memory (LSTM) networks and bi-directional recurrent neural networks (Bi-RNN). Evaluation of the obtained models was performed both automatically and manually. For automatic evaluation, the bilingual evaluation understudy (BLEU) score was used, and for manual evaluation, three metrics such as adequacy, fluency, and overall ranking were used. Analysis of the results showed the presence of lengthy sentences in English–Malayalam, and the English–Hindi corpus affected the translation. Attention mechanism was employed with a view to addressing the problem of translating lengthy sentences (sentences contain more than 50 words), and the system was able to perceive long-term contexts in the sentences. Similarity profiled association mining from time stamped transaction databases is an important topic of research relatively less addressed in the field of temporal data mining. Mining temporal patterns from these time series databases requires choosing and applying similarity measure for similarity computations and subsequently pruning temporal patterns. This research proposes a novel z-space based interest measure named as Krishna Sudarsana for time-stamped transaction databases by extending interest measure Srihass proposed in previous research. Krishna Sudarsana is designed by using the product (...) based fuzzy Gaussian membership function and performs similarity computations in z-space to determine the similarity degree between any two temporal patterns. The interest measure is designed by considering z-values between z = 0 and z = 3.09. Applying the Krishna Sudarsana requires moving the threshold value given by user to a different transformation space which is a defined as a function of standard deviation. In addition to proposing interest measure, new expressions for standard deviation and equivalent z-space threshold are derived for similarity computations. For experimental evaluation, we considered Naïve, Sequential and Spamine algorithms that applies Euclidean distance function and compared performance of these three approaches to Z-Spamine algorithm that uses Krishna Sudarsana by choosing various test cases. Experiment results proved the performance of the proposed approach is better to Sequential approach that uses snapshot database scan strategy and Spamine approach that uses lattice based database scan strategy. As soon as we broach the subject of aesthetics, we are already in the realm of ideology; as soon as we impose the frame of scientific inquiry upon any subject, we invoke another kind of ideology. The previous issues in this series have depicted the unfolding of an ideological clash of cultures between sciences and the humanities, enough to make C.P. Snow blush. Rationale, aims and objectives To compare the pharmacotherapeutic approaches to diabetic hypertension of family physicians (FPs) and general practitioners (GPs). Methods A retrospective prescription-based study was conducted in 15 out of a total of 20 health centres, involving 115 primary care physicians – 77 FPs and 38 GPs, representing 74% of the primary care physicians of Bahrain. Prescriptions were collected during May and June 2000 to comprise a study population of 1266 diabetic-hypertensive patients. Results As monotherapy, angiotensin-converting enzyme (ACE) inhibitors (...) (37.9%) and β-blockers (38.3%) were the most commonly prescribed classes of antihypertensives by FPs and GPs, respectively. Calcium channel blockers (CCBs) were ranked third by both categories of physicians. For two-drug combinations, a β-blocker and an ACE inhibitor was the combination of choice for both physician categories. Patients managed by the FPs were more likely to receive a β-blocker–CCB combination (17.4 vs. 14.9%) or a diuretic–ACE inhibitor combination (16.7 vs. 11.4%) and less likely to receive a β-blocker–diuretic combination (11.8 vs. 16.7%) than those managed by the GPs. The proportion of patients receiving antihypertensive combinations was 40.6 and 38.5% for FPs and GPs, respectively. While the GPs prescribed CCB as a monotherapy to the elderly most often, the FPs choice was a β-blocker. Diuretics were less preferred by both FPs and GPs. β-Blocker–ACE inhibitor was again the most preferred combination of both FPs and GPs. FPs prescribed CCB–β-blocker combinations more often than GPs (P = 0.01), whereas CCB–ACE inhibitor combinations were less preferred (P = 0.09). A trend towards excessive use of short-acting nifedipine as monotherapy for elderly patients, both by FPs and by GPs, was noticed. Glibenclamide, alone or in combination with metformin, was the foremost antidiabetic drug prescribed by FPs and GPs. Middle-aged (45–64 years) patients seen by GPs were more likely to receive glibenclamide than those treated by FPs (P = 0.001) and less likely to receive gliclazide (P = 0.01). Combinations of a β-blocker with either glibenclamide or insulin were prescribed considerably more often by GPs.Conclusions Within the same practice setting, a substantial difference was observed between FPs and GPs in terms of preference for different classes of drugs in the management of diabetic hypertension. The compliance of both FPs and GPs was suboptimal; overall, the compliance of the FPs was closer to the recommended guidelines, however. Educational programmes should specifically address these inadequacies in order to improve the quality of health care. (shrink)

Crop genetic resources constitute a ‘new’ global commons, characterized by multiple layers of activities of farmers, genebanks, public and private research and development organizations, and regulatory agencies operating from local to global levels. This paper presents sui generis biocultural community protocols that were developed by four communities in Benin and Madagascar to improve their ability to contribute to, and benefit from, the crop commons. The communities were motivated in part by the fact that their national governments’ had recently ratified the (...) Plant Treaty and the Nagoya Protocol, which make commitments to promoting the rights of indigenous peoples, local communities and farmers, without being prescriptive as to how Contracting Parties should implement those commitments. The communities identified the protocols as useful means to advance their interests and/or rights under both the Plant Treaty and the Nagoya Protocol to be recognized as managers of local socio-ecological systems, to access genetic resources from outside the communities, and to control others’ access to resources managed by the community. Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk.
